My latin is also a bit rusty, but I think the last line is "uxor Johannis Piper seni sepultas" possibly "wife of the late Johanis Piper"? (sepultum means to bury)
Also, I think "decimo tertio" means "thirteenth".

My latin does not go much better than Amo, amas amat ( and I probably got that wrong)
I think it says

before Johannis Piper ???? ???? (priests name?)
