Given the return address, it is probably a coil dispensing machine that is out of register with the coil roll.

Roy is correct, the dispensing machines often screwed up, many used 'grippers' to advance the coil stamp to the cutter. I have the example below which looks to be a 'bi-sect' usage but really is not, it is simply a misfeed from the dispensing machine.
If you look closely, you can see the grippers have actually damaged the stamp when it tried to pull the coil roll forward. I think this happened when the coil roll got close to the end of the roll.
